Prerequisites

• Guidelines PSN- PSR for local services ;
• Principles of health education : the WHO projects for the promotion of health ;
• Health education: history, principles , models , strategies and interventions ;
• Management nursing : the educational role of the nurse in the field of care and theories of reference;
• Skills and methods of nursing intervention in the community ' ;
• Chronic care model and desease management;
• Role and function of preventive , educational , charitable community nurse : the educational process applied to the assisted person ;
• History of therapeutic ; bases of therapeutic relationship , the concept of therapeutic alliance , compliance and adherence ;
• The methodology of therapeutic education : tools and monitoring .
• Counseling nursing and the helping relationship : the relational modalities designed to support the person in identifying realistic expectations and to integrate the experience of illness within their life.
• The nurse case managers in primary care : the network of hospital and territorial services with varying degrees of care for the continuity of takeover .. Discharge protected and assisted , home hospitalization .
